Welcome! At Accent Women's Health, we recognize your appointment day is not always a day women look forward to. However, it is our opportunity to fulfill your every expectation. Our goal is to make your time in our office as enjoyable as possible and concentrate on resolving your healthcare concerns.

To simplify the process of making appointments, you can fill out the registration forms ahead of time. If you are a new patient or there are changes in your demographic or insurance information since your last visit, please download the Patient Registration Forms below. Complete and sign the forms, and then bring them with you to your next appointment. If you cannot access the forms and would like them faxed or mailed to you, please contact one of our office personnel. They will be glad to send you the appropriate forms.

Call for Your Appointment

Please call (479)444-1440 to schedule an appointment. If we do not have your information, or if your information has changed, please notify us at this time and have your insurance information available. If there is a particular time or day of the week that is convenient for you, please let us know. We will do our best to accommodate your schedule.

Your First Visit

Normally, we ask you to arrive 30 minutes prior to your appointment time in order to complete the necessary paperwork. However, if you have completed the Patient Registration Forms in advance, please arrive 15 minutes prior to your appointment time. If you have not filled out the patient paperwork, it will be located next to the sign in sheet on the reception table. Please take a clipboard and begin to fill out the paperwork before the appointment.

For new patient appointments we do allow extra time for you to get to know your physician and nurse. We care that there is ample time to address all of your questions and needs. This is one of the reasons we ask you to arrive early for these appointments as they take extra time.

Your Appointment Day

Please bring your current insurance card to your appointment. If you have already provided your insurance card at a previous appointment and we have scanned a copy into our system, this is not necessary unless the information has changed. If your demographic information has changed since the last time you visited our office, please do not feel the need to fill out the Patient Registration Forms again. Please notify the attendant who brings you to the exam room and she will be glad to provide you with a patient update form. Please also take note to come prepared to pay your applicable co-pay and/or deductible at the time of service.

Frequently Asked Questions

Q. When should I schedule my annual exam?

A. Most insurance companies will pay for an annual or well woman exam once per year, so it is important to schedule your visit at least one day after the date of your last annual or well woman exam. For your convenience, our office will mail you a reminder letter with that date.

Q. What if I am running late for my appointment?

A. Please call the main number (479-444-1440) and notify us of the delay. Usually, if you are more late than our schedule allows us for that day, we will need to re-schedule your appointment to the next possible time.

Q. Will you remind me of my appointment?

A. As a courtesy to you, someone in our office will call to remind you of your appointment on the day before. If we get your voice mail or another member of the household, we will leave a message only that we are confirming an appointment and the time. We will not leave detailed appointment information to anyone other than the patient.

Q. Should I bring my records from my previous doctor's office?

A. If you are transferring with a chronic medical condition, please bring your records with you on your first visit. If you are not having an ongoing problem or do not need the records from a different clinic for Dr. Bradford to see, we will request what is necessary when you come in for your appointment and retrieve those for you at that time.
