Insurance can be extremely confusing! We are committed to helping you with any insurance or billing concerns that you may have. Please contact our office with questions at 479-444-1440. We will take your question, but may not be able to resolve your concerns immediately. Sometimes, an associate may have to look into the problem or contact the particular insurance company to recieve an answer to your concerns.

Insurance and Billing Information

Dr. Bradford participates in most managed care plans. If we participate with your plan, we will file a claim to them for the services we provide to you. You are responsible for any co-payment and/or deductible at the time services are rendered.

If we do not participate with your insurance plan, we will expect payment at the time services are provided. Our experience is that this method works best for both parties.

We accept cash, check, Visa, and MasterCard.

Fees

We are aware of present health care costs and make efforts to keep our fees reasonable. They are within the customary range for this area and reflect the appropriate level of care you will receive. If you have any questions about fees, please feel free to call and discuss them with our Billing Department at 479-444-1440.

Insurance Verification

Before most procedures and all surgeries, the billing department verifies your eligibility and benefits to determine your financial responsibility. This allows us to know in advance what your insurance company will pay for services to come. Therefore, we will collect that amount from you at the time of your procedure, or the appointment before your surgery. While we provide this verification service to you as a courtesy, we are also quoted by your insurance company that the benefits given are not a guarantee of payment. We must advise you that it is ultimately the patient's responsibility to be aware of coverage for any medical service received. Therefore, we strongly advise that you contact your insurance company prior to your visit to verify that the service for which you are scheduled is covered under your plan.

Q. Do you accept my insurance?

A. We accept most insurance plans but to ensure that we participate with your insurance, we encourage you to contact your insurance company directly to find out if our physician participates with your plan.

Q. I am getting information about Medicare HMO's. Are you a provider?

A. Currently we are not providers with any Medicare HMO's. We accept Medicare Part B only.

Q. Why do you need a copy of my insurance card?

A. We need a copy of your insurance card to insure that the correct information is obtained to file your claim.

Q. What happens if I arrive at my appointment without my insurance card?

A. You may be asked to reschedule your appointment or pay in full for your visit if you cannot retrieve your insurance information.

Q. Do I have to provide your office with my insurance coverage through my employer even if I choose to only use my spouse's insurance for reasons of better coverage?

A. Yes, you have to provide our office with your primary insurance through your employer. All medical services have to be filed with your insurance coverage through your primary insurance. If claims are filed under a secondary, they will be denied by the insurance company and they will inform us to file with your primary first. Delays in timely filing of insurance may result in denial from your insurance company in which case you will have to pay for the services.

Q. Should I contact your office if my insurance has changed or if I am no longer insured?

A. Yes, you should contact our billing office with any insurance changes or cancellation of coverage. Failure to inform us of any changes may result in you having to pay in full for your medical services.
